The Anne Arundel County Mental Health Agency, Inc. (AACMHA) works in partnership with the Behavioral Health Administration and the Substance Abuse and Mental Health Services Administration (SAMHSA) on the SSI/SSDI Outreach, Access and Recovery (SOAR) model, through its PATH program. 

The SOAR model is designed to increase access to disability income benefits for adults experiencing homelessness or at imminent risk of homelessness and have and severe mental illness (SMI) or co-occurring SMI and substance use disorder. Individuals experiencing homelessness or at imminent risk of homelessness have significant barriers to accessing Supplemental Security Income and/or Social Security Disability Income (SSI/SSDI) benefits. The SOAR model works to assist individuals experiencing homelessness or at imminent risk of homelessness in overcoming those barriers to secure safe and affordable housing, improve their health and live self-directed purposeful lives in Anne Arundel County.


Individuals experiencing homelessness or at imminent risk of homelessness experience trauma and are impacted by trauma at higher rates than the general population. Outreach and engagement through the SOAR process are key components to the success of the model. SOAR links this vulnerable population to mainstream benefits and other supportive services in Anne Arundel County.

Scope of Work
The Anne Arundel County Mental Health Agency (AACMHA) seeks proposals for a vendor to implement the SOAR model in Anne Arundel County. The SOAR model uses five critical components in gathering complete, targeted, and relevant information for Social Security Administration (SSA), Disability Determination Services (DDS), resulting in high-quality SSI/SSDI applications.

Pre-Bid Conference
A pre-bid conference will be held on Tuesday, May 6, 2025 at 10 am via Zoom Video Conference. The purpose of the conference is to address questions concerning the expectations of the project.
