Anne Arundel County Mental Health Agency Request for Proposal (RFP) for SSI/SSDI Outreach, Access, and Recovery (SOAR)
The Anne Arundel County Mental Health Agency, Inc. (AACMHA) works in partnership with the Behavioral Health Administration and the Substance Abuse and Mental Health Services Administration (SAMHSA) on the SSI/SSDI Outreach, Access and Recovery (SOAR) model, through its PATH program.
The SOAR model is designed to increase access to disability income benefits for adults experiencing homelessness or at imminent risk of homelessness and have and severe mental illness (SMI) or co-occurring SMI and substance use disorder. Individuals experiencing homelessness or at imminent risk of homelessness have significant barriers to accessing Supplemental Security Income and/or Social Security Disability Income (SSI/SSDI) benefits. The SOAR model works to assist individuals experiencing homelessness or at imminent risk of homelessness in overcoming those barriers to secure safe and affordable housing, improve their health and live self-directed purposeful lives in Anne Arundel County.
Individuals experiencing homelessness or at imminent risk of homelessness experience trauma and are impacted by trauma at higher rates than the general population. Outreach and engagement through the SOAR process are key components to the success of the model. SOAR links this vulnerable population to mainstream benefits and other supportive services in Anne Arundel County.
Scope of Work
The Anne Arundel County Mental Health Agency (AACMHA) seeks proposals for a vendor to implement the SOAR model in Anne Arundel County. The SOAR model uses five critical components in gathering complete, targeted, and relevant information for Social Security Administration (SSA), Disability Determination Services (DDS), resulting in high-quality SSI/SSDI applications.
Pre-Bid Conference
A pre-bid conference will be held on Tuesday, May 6, 2025 at 10 am via Zoom Video Conference. The purpose of the conference is to address questions concerning the expectations of the project. Anne Arundel County Mental Health Agency Request for Proposal (RFP) for Transition Age Youth Program (TAY)
The Transition Age Youth Program provides a system of care to help youth and young adults smoothly and successfully transition into adulthood. The purpose of the TAY program is to provide the individual with positive, appropriate, and relevant services and support that engage and motivate the individual.
The TAY program promotes self-sufficiency, resiliency, recovery and encourages success in areas imperative to future well-being. These areas include enhancing educational skills, employment skills, social skills and connecting TAY participants to local community activities. Flexible funding is available to support many of these activities.
The Anne Arundel County Transitional Age Youth program serves youth and young adults aged 16-26, in the Public Behavioral Health System. The TAY program is outcome based with assessments completed at regular intervals to assess the individual’s progress. The TAY staff work with youth to tailor the goals and interventions to help participants successfully transition into adulthood. The TAY program provides opportunities for participants to develop social skills, basic life skills and provide opportunities to engage in activities and explore career and educational interests.
While the TAY program’s primary focus is youth, the TAY staff also work with the individual’s families and others important to the individual to provide psychoeducational support. The TAY staff assist in identifying or developing a support system for the individual. With appropriate releases, the family members are consulted and kept informed throughout the individual’s involvement with the Transitional Age Youth Program. The TAY program is intended to be an adjunct service, with therapy being the primary service. Goals are determined in conjunction with the youth, family and the team. Monthly contact is maintained between the individual’s therapist and the TAY Staff in order to provide each other with progress on goals. For the youth and young adults, services are coordinated between the treatment team and the individual’s school, as appropriate. TAY staff also assist youth and the individual’s family, as needed, in obtaining benefits to help the individual achieve independence.
The TAY program staff will be expected to enroll and complete training by the University of Maryland.
Scope of Work
The Anne Arundel County Mental Health Agency is seeking a single provider to serve the TAY population in Anne Arundel County who meet the standards included in the:
• Federal Medicaid requirements and State Medicaid Plan requirements for the type of service.
• Proposal Requirements of the Anne Arundel County Mental Health Agency, Inc. and
• Statements made in the reply to this RFP.
The AACMHA will oversee and monitor compliance with all contract requirements to ensure procedural requirements and contract deliverables are met. The offeror shall ensure that the AACMHA will have full access and copies of any and all materials to fulfill this contract oversight role. This should include but is not limited to individual records, case ratios, staffing levels and patterns, organizational parameters, service requirements budget and financial records.
